If you are wanting to join our practice and live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.
You will need to collect and complete a registration pack and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients will be registered as soon as these forms are fully completed. You will need to provide your nhs number which you can obtain from your old surgery and some form of PHOTO IDENTIFICATION. You will then be required to have a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
If you cannot attend for your appointment please let us know in advance so we can re-arrange this for you.
Medical treatment is available from the date of registration. Please contact reception for further information.
It is a good idea if new patients arrive a few minutes before their first appointment time, so we can put all your details on our computer system.
